Established practice for recording hazardous actions

Minor violations
The most massive category: unfastened hard hat, missing gloves, brief presence in a hazardous zone, or passing under a suspended load without consequences. These violations are often ignored or not recorded, but it is precisely these, in the absence of a response, that escalate into more serious events. The system's task is to record minor violations to correct personnel behavior in time
Moderate incidents
Events leading to minor or moderate injuries (bruises, cuts, fractures, minor burns) or material damage (equipment breakdown, emergency shutdown). May require medical assistance but do not lead to disability or death. Task: identify causes common to minor violations and block them before more severe incidents occur
Fatal incidents
The rarest but most catastrophic events. As a rule, they cannot be completely prevented without analyzing and managing the lower levels. Recording only fatalities without analyzing minor violations is ineffective. The practice implies continuous data collection at all three levels to affect the roots of dangerous behavior

Audio-visual signaling

The system automatically detects the presence of a person in a zone where equipment operation creates a threat to life or health (under moving parts of a mill, near conveyors, near open hatches, within the operating radius of robots, etc.). As soon as personnel cross the boundary of a hazardous zone, audio-visual signaling is instantly activated — a bright flashing lamp (usually red) and a loud intermittent sound. The signal remains active until the violator leaves the zone. Each event is recorded in a log with time and location tagging. Data is transferred to the corporate security analysis system for subsequent prevention of more serious incidents
